Primary records lead.
Case files begin with public investigation material from OSHA, NIOSH FACE, state FACE programs, or another authoritative source. FACE means Fatality Assessment and Control Evaluation.
Say only what the record supports
Event details, contributing factors, and recommendations are paraphrased from the cited source. Every case includes a source ledger so readers can inspect the record directly.
Do not turn possibility into fact
If the initiating event was unwitnessed or investigators considered several explanations, the page preserves that uncertainty instead of selecting the most dramatic version.
AI visual standard
Anonymous means untraceable to a real person.
The site is faceless by design. Morbid Safety does not use the editor’s on-camera identity, recreate an actual victim’s face, or present AI imagery as documentary evidence.

Identity standard
Teach the failure—not the identity.
Names, recognizable faces, employer branding, and precise workplace identities are omitted from Morbid Safety’s narrative unless they are essential to understanding the public record. Linked government reports may contain details that the case page intentionally leaves out.
Prevention standard
Controls before equipment.
The strongest available control is explained first. A case will not force a PPE or equipment recommendation when the source points to elimination, de-energization, guarding, a different work method, or an engineered system.
Measured and conditional
Equipment may “address the identified hazard” or “reduce risk when properly selected and used.” It is never described as guaranteed protection or as something that certainly would have saved a worker.
Exact links, flexible recommendations
Morbid Safety may add products as new hazards and tasks are covered. Each specific link must match the reviewed model and hardware configuration. Harness sizing remains wearer-specific.
Commerce standard
Credibility comes before commission.
Affiliate relationships, if activated, will be disclosed in plain language near the recommendation. Commission rate does not choose the product, alter the investigation, or move equipment ahead of the control.
